I, an adult, on behalf of myself as well as on behalf of my organization/team, coaches, any of my own children or their teammates who are to be considered my wards as well as wards of our team/organization, as well as anyone who accompanies me or my teams to any events (collectively, "Releasors"), hereby give consent for Releasors to participate in or attend activities at the premises of HOOPREVIEW, LLC ("HOOPREVIEW") or other premises hosted or co-hosted by HOOPREVIEW or its event partners or coordinators. Releasors declare that the members of the organization/team are in good physical condition. Releasors acknowledge and agree that attending or participating in sports may be hazardous and may result in injury. Releasors agree to assume all risks of injury that may be incurred or suffered while upon the premises (premises include all areas of the building(s) and any outdoor parking, etc.) or as a result of using the facilities or equipment therein.
Releasors expressly agree to release HOOPREVIEW, its owners, employees, agents, successors, assigns, affiliates, landlords/building owners, insurers, and anyone else associated with HOOPREVIEW as well as any independent contractors, partners or co-hosts assisting with the activities or otherwise on the premises (including without limitation any premises where events are held, and any lessors or owners of premises where events are held), from any and all claims, demands or damages whatsoever, whether developed or undeveloped, known or unknown, anticipated or unanticipated, have, now or in the future, including, but not limited to any and all claims, demands or damages for negligence, personal injury and/or loss, theft or destruction of personal property. It is Releasors' intention that this release be as broad as California law allows releases of this sort to be.
Releasors give HOOPREVIEW the right to copyright and/or publish, reproduce, or otherwise use my players' name, voice, and likeness and/or photographs, and audiovisual recordings that include Releasors for instruction, advertising, program website, publications or brochures, or any other lawful purpose whatsoever. Releasors hereby agree to relinquish all rights, title and interest if any to the finished product and waive all rights to any compensation thereof.
Releasors agree to save, hold harmless, and indemnify HOOPREVIEW, HOOPREVIEW, its owners, employees, agents, successors, assigns, affiliates, landlords/building owners, insurers, and anyone else associated with HOOPREVIEW as well as any independent contractors, partners or co-hosts assisting with the activities or otherwise on the premises (including without limitation any premises where events are held, and any lessors or owners of premises where events are held), from any and all claims, demands or damages, including costs, interest and attorneys' fees which they may suffer or incur as a result of any claims by me, my team/organization, anyone who comes with me to the premises or any Releasors, or related entities, and/or as a result of any claims, demands or lawsuits arising out of my actions or those of anyone who comes with me to the premises or any Releasors.
Payment of this invoice and/or attendance at an event and/or participation in activities confirms that Releasors agree to the above terms and conditions and that the person paying this invoice and/or attending the activities has the authority to bind Releasors, including him or herself, their organization/teams, their own children and any teammates who are to be considered wards of the organization/team and their coaches.
Behavior Guidelines
HOOPREVIEW does not allow disrespectful, profane, or improperly aggressive behavior towards officials, players, coaches or spectators, and reserve all rights to ask any player, coach or spectator to leave events without a refund.

I am aware of and informed of the risks of concussions in youth sports and as a coach and/or parent have discussed this with the player(s).
